Insurance & Fees

One of the values of Midwest DBT is making high quality DBT as accessible as possible. This means accepting insurance for all possible services, being fully transparent with regards to our out of pocket pricing, walking clients through the process of out-of-network reimbursement, and considering sliding scale need whenever possible.

We are in network with the following insurance carriers for Illinois residents:

  • Blue Cross Blue Shield PPO

  • Blue Choice

  • Aetna

*If using insurance, we strongly recommend calling your carrier before starting treatment to confirm we are in-network. If you are in need of a list of the CPT Codes we would use for billing, please ask and this can be provided to you.

If we are not in network with your insurance, we recommend asking your carrier about your out of network benefits. We can provide you with a monthly “Superbill” reflecting what you have paid each month, which you can submit to your insurance carrier for reimbursement.

If you are unable to use insurance and cannot afford our out of pocket rates, please inquire about our process for applying for sliding scale services.

  • Comprehensive, 50-55 minute intake assessment. This is the session each client presenting to treatment starts with. This session is a chance for us to get to know each other, me to learn about what you are looking for in therapy, and to share with you about our different treatment options. There is no expectation that you proceed with treatment following this, it is simply a chance to learn about what we offer and, if relevant, to find out how long our waitlist is/when you can expect to start.

    Out of Pocket Fee: $245

  • This is the first session of your course of treatment. It will include additional assessment, treatment planning, organizing your goals, and answering any additional questions about your treatment options.

    Out of Pocket Fee: $245

  • These are your ongoing, regularly scheduled individual therapy sessions. They are 50-55 minutes long and may be either DBT, supportive psychotherapy, or individual skills training.

    Out of Pocket Fee: $210

  • Any sessions where you meet 2-1 with your provider. These may be parent coaching sessions or couple therapy, and are 50-55 minutes long.

    Out of Pocket Fee: $225

  • These sessions include the primary client and 1 or more family members. They are 50-55 minutes long.

    Out of Pocket Fee: $245

  • Extended sessions are recommended in the course of DBT-Prolonged Exposure Therapy treatment for PTSD. The out of pocket rates are pro-rated as follows:

    90 minutes: $305

    120 minutes: $420

  • This service can include any type of letter you request from your provider. Examples include: IEP Letters, Accommodations Letter, etc. Not covered by insurance.

    Out of Pocket Fee: $160

  • This service can include any type of paperwork you request from your provider. Examples include FMLA or disability services form completion. Not covered by insurance.

    Out of Pocket Fee: Pro-rated for time spent, based on $160/hour.
